The purpose of this visit was to conduct a follow-up inspection on the CFM card and the 2015 permit. Permit is on the way and CFM picture card was observed. CFM from routine inspection still needs to send a copy of her ID.
  • Permit to Operate Required, Must be Posted, Must be Valid (repeated violation)
    Observation: The food establishment's Permit to Operate is no longer valid due to expiration of the permit on December 31, 2014. No person shall operate a food establishment unless that person possesses a valid permit issued by the Health Department.
    Correction: To obtain a current Permit to Operate, contact the Health Department at (703) 246-2444 within 48 hours to make payment of the permit renewal fee. You may provide payment by cash, check, or credit card. Failure to pay the permit renewal fee and obtain a current Permit to Operate within 48 hours may lead to further enforcement actions by the Health Department including immediate closure.

    Observation: There was no temperature measuring device located in the following cold or hot holding equipment: randell 3dr preptop cooler
    Correction: Cold or hot holding equipment used for the storage of potentially hazardous foods (time/temperature control for safety food) shall be designed to include and shall be equipped with at least one integral or permanently affixed temperature measuring device that is located to allow easy viewing of the device's temperature display.

    Observation: The person in charge (PIC) has a valid certificate of successful completion of a Certified Food Manager (CFM) exam but does not possess the CFM card issued by ORS Interactive, Inc.

    Correction: Handsinks are to be used only for washing hands to encourage proper hand washing and to prevent contamination of cleaned hands.

    Observation: Observed that sign or poster that notifies food employees to wash their hands is not provided at bathroom handwashing sink used by food employees.
    Correction: Provide a sign or poster at all handwashing sinks used by food employees that reminds food employees to wash their hands.

  • Responsibilities of Owner or Proprietor
    Observation: Absence of no-smoking signs or international no-smoking signs in non-smoking area.
    Correction: Post of no-smoking signs or international no-smoking signs in non-smoking area. These can be found online at www.vdh.virginia.gov/breatheeasy
  • Food Storage Containers, Identified with Common Name of Food (corrected on site)
    Observation: The following food items that are not easily identified by appearance were observed without a label: Flour container and corn meal container.
    Correction: Unless a food item can be easily recognized, all foods and ingredients removed from their original containers and stored in working containers shall be labeled using the common name of the food. For example: cooking oils, salt, sugar, flour, spices, herbs, etc. MANAGER LABELED CONTAINERS.

    Observation: Observed that sign or poster that notifies food employees to wash their hands is not provided at all handwashing sinks used by food employees. THERE ARE NO HAND WASHING SIGNS POSTED IN THE RESTROOM.
    Correction: Provide a sign or poster at all handwashing sinks used by food employees that reminds food employees to wash their hands. EHS PROVIDED HAND WASHING SIGN TO POST IN THE RESTROOM.
07/02/2014Routine
The purpose of this visit is to conduct a complaint inspection. The general manager was not aware of the complaint that an employee with an open wound was working in the food preparation area. She was not handling food but was moving between the cashier area and the food preparation area. The customer was concerned that she may need to cover the lacerations on her arm. The general manager stated that she does not have an employee to match the description provided by the customer and they have an employee health policy stating the frequency of hand washing and covering wounds. At the time of the inspection this complaint was not confirmed.

The purpose of today's visit was to conduct a routine inspection. All items are received pre-cooked. Pizzas are assembled and timer is used to track, cooked within 1 hour and then items are kept hot and under TPHC.
Facility is categorized as low priority based on risk assessment and will be inspected once per year, minimum.

4. Ceiling tiles have not been installed at kitchen, rest room and at front serving area.
5. Waste water drain line from the grease interceptor drains into an open site. Install a DIRECT/HARD connection between the grease interceptor and main sewer line.
6. Covered trash receptacle has not been provided at rest room.
7. A dumpster has not been provided.
