    Observation: Raw shrimp stored over ready-to-eat (RTE) cocktail sauce in the refrigeration unit.
    Correction: Separate raw foods of animal origin during storage, preparation, holding, and display from raw RTE food including foods such as fish for sushi or molluscan shellfish, or other raw RTE food such as vegetables, and cooked RTE food.
  • Equipment - Good Repair and Proper Adjustment (corrected on site)
    Observation: Prep unit is not maintaining food at 41 F or less.
    Correction: Adjust the prep unit so that it will maintain 41 F or less.

Recent Plans for the cigar room have been reviewed and approved. The fire marshall's most recent inspection was 8-4-15. The certificate. of occupancy is from November 6, 2014.
No violation noted during this evaluation.		08/05/2015Routine
Met with the GM, Nate Alexander. He was given the Food Permit for the operation of Ray's Night Club. The following items were discussed and noted:
(1) For Ray's Night Club that has been in operation since January 23, 2015, a Food Permit is required. Based on the inspection of January 23, 2015, this Food Permit is given. This permit is to be posted in public view. It was noted that the conditions that were given as a result of the January 23, 2015 inspection still applied namely: (a) Use of the Front Line Area (absolutely no use of cooking equipment inside of the Main Kitchen Area is permitted prior to approval from the local health Department, Fire Department, and other agency where approval is required

The following items/actions are temporarily permissible until 28-February-2015 by which date the operator must contact the local health department to schedule an inspection for the purpose of obtaining a permanent annual food permit:
-preparing, storing, holding, cooking, reheating, cooling, and serving food items
-washing, rinsing, and sanitizing equipment and utensils as stated in local and state food regulations
-use of the FRONT LINE AREA ONLY (ABSOLUTELY NO USE OF COOKING EQUIPMENT INSIDE OF THE KITCHEN AREA IS PERMITTED PRIOR TO APPROVAL FROM THE LOCAL HEALTH DEPARTMENT, FIRE DEPARTMENT, AND OTHER AGENCY WHERE APPROVAL IS REQUIRED)
-use of the FRONT LINE AREA WITH THE EXCEPTION OF THE TWO END APPLIANCES (FRYER AND GRILL) PER THE NEWPORT NEWS FIRE DEPARTMENT (12/31/14) IS PERMITTED
No TCS food items shall be cooked/served undercooked nor raw as there is no appropriate consumer advisory on menus. The operator shall replace the light bulb that is out underneath the front line ventilation hood system.
  • Hand Drying Provision (corrected on site)
    Observation: No disposable towels were provided at the hand washing lavatory on the front line.
    Correction: Hand drying devices such as individual disposable paper towels, a continuous towel system that supplies the user with a clean towel or heated air hand drying device must be provided at all hand washing lavatories to encourage proper hand washing and avoid employees to drying their hands on their clothing or other unclean materials
  • Premises - Maintaining Premises - Unnecessary Items and Litter
    Observation: Facility is storing unnecessary items to the operation or maintenance of the establishment.
    Correction: Remove all unnecessary items from the facility. The presence of unnecessary articles, including equipment which is no longer used, makes regular and effective cleaning more difficult and less likely. It can also provide harborage for insects and rodents.
