The purpose of this visit was to conduct a routine risk evaluation. Dishmachine turned thermolabel black. Quaternary ammonium sanitizer solution concentration in wiping cloth bucket/ 3 vat sink: 200/300 ppm. NOTE: PLEASE REMEMBER TO PROVIDE COPIES OF EMPLOYEE ILLNESS REPORTING AGREEMENTS TO HEALTH DEPARTMENT WITHIN 14 DAYS AS REQUIRED ABOVE.
- Critical: Responsibility of PIC to Require Food Employees to Report GI Symptoms or Cuts, Disease, Medical Condition or Exposure to PIC (repeated violation)
Observation: Upon discussion with the person-in-charge, one or more of the elements of an effective employee health policy is either missing or incomplete. A complete employee health policy is required to be in place at the food establishment. At the time of this inspection, the PIC provided a copy of the "Employee Health Policy." Signed employee illness reporting agreements had not been updated since August 2013. NOTE: PIC will provide copies of signed employee illness reporting agreements for all current employees to Health Department within 14 days.
Correction: A complete employee health policy must have the following elements:
1) Employee training on foodborne illness, particularly symptoms of illness and prevention of the Big Five illnesses (see "What is Foodborne Illness" handout)

The purpose of this visit was to conduct a routine risk inspection. Water heater information: Rheem 675-125. Dish machine: Hobart LXi - turned thermolabel. Quaternary ammonium sanitizer in 3 vat sink: 300 ppm. PIC advised pest control service is provided monthly. PIC to send statement to Health Department by 3/22/13 advising that all employees have been trained in EHP requirements, and all that employees have signed an employee reporting agreement. PIC to include copies of employee reporting agreements for all current employees.

- Utensils That are In-Use / Between-Use Storage
Observation: Dispensing and/or in-use utensils improperly stored between use as follows: ice scoop on top of ice machine.
Correction: During pauses in food preparation or dispensing, food utensils shall be stored in one of the following manners: 1) in the ice with their handles above the top of the ice, 2) in a clean, protected location.
- Critical: Cold Holding of Potentially Hazardous Food at 41°F or less
Observation: The following food item(s) were observed cold holding at improper temperatures using a calibrated food temperature measuring device: whole milk thermos carafe at customer coffee prep counter.
Correction: Potentially hazardous foods (time/temperature control for safety food) shall be held cold at a temperature of 41°F or below unless the permit holder is using "time as public health control" as specified under 3-501.19 to limit bacteria growth.
- Critical: Quaternary Ammonium Compound Sanitizer Solution Concentration per Use Directions (repeated violation)
Observation: When tested, the quaternary ammonium compound-based sanitizing solution was observed to be at a concentration less than what is required for effective sanitization of food-contact surfaces of equipment and utensils. Observed the sanitizing solution in the wiping cloth containers at coffee station with a concentration of < 150 ppm total quaternary ammonium compound.
Correction: The quaternary ammonium sanitizer used in this facility shall have a minimum concentration of at least 200 ppm OR a concentration between 150-400ppm based on the manufacturer's use directions and using the appropriate test kit.
- Hand Drying Provision / Individual, Disposable Towels
Observation: Observed that paper towels were not provided at each handwashing sink. Handwashing sink near back door.
Correction: Provide paper towels at each hand sink to allow employees to properly dry their hands after handwashing.
