Note: This food establishment has added a hand sink located next to the 3-compartment utensil washing sink. Please provide soap and a paper towel dispenser along with a sign to designate this sink for hand washing only.

| This visit was made to conduct a change of ownership evaluation. The following items shall be corrected within 30 days: 1. Provide a thermometer for the Continental undercounter refrigerator 2. Caulk women's restroom hand sink. 3. Provide a covered garbage receptacle for the women's restroom. 4. Increase lighting for both the women and men's restroom (currently 5 foot candles in women and 1 foot candle in men). 5. Repair or replace damaged floor sweep for back door 6. Remove unnecessary items from food establishment (old paint cans). Note: Remove RAID pest control can from premises and remember to store chemicals below and away from food and food contact surfaces. Note: Any changes to food menu or equipment must be pre-approved by the Alexandria Health Department. Hot water for kitchen hand sink: 126 degrees F Letter for insufficient hot water signed by owner. See file. Note: Food Safety Information binder provided to food establishment during the pre-opening evaluation. Discussed with the Food Protection Manager (FPM) major changes with the new 2013 Food Code to include employee health, clean-up of vomiting and diarrheal events, food allergens, responsibilities of permit holder (sticker provided), and increases in civil penalties. --Approved for change of owner recommend issuance of Alexandria Health Department permit.
