Two #10 cans of hominy were severely dented and stored with wholesome cans in isle 3. Moldy strawberries were present in the Gazebo area.
Beef jerky was prepared in the facility without an approved HACCP plan and stored at room temperture.
A consumer advisory was not posted.
An employee beverage was present in the Gazebo area.
A faucet able to disperse water into all 3-compartments of the dish washing sink was not installed.
The required atmospheric (hose bib) vacuum breaker was not installed on the utility sink in the men's restroom.
A handsink was not installed by the bulk ice cream cone freezer.
Soap and paper towels were not available at the Gazebo and Meat Market handsinks.
Rodent droppings were observed on the bottom storage shelving across from the mop sink.
Unapproved rodent pellets were present on the floor to the left of the ice machine.
Glass cleaner was stored next to plastic food containers on the shelving in the Meat Market.
An unlabeled container filled with a salt mixture was present in the Meat Market.
Customers had access to the tamale freezer at the rear of the facility.
Plastic food containers were stored on porous wood shelving in the Meat Market. Aluminum foil was used to line shelving in the wait station area. The shelving was constructed out of porous wood in the Produce walk-in cooler. A wooden cutting board was stored by the microwave in the Gazebo. Two non food grade tables were present in the Gazebo area. The glass was cracked on the far right meat case.
The laminate was chipped on the red shelving in the kitchen. The microwaves were stored on porous wood and cardboard. The caulking was cracked on the large 3-compartment sink. The preparation sink was not securely sealed to the wall. A plastic bag was wrapped around the preparation sink faucet. The light shield was cracked on the Bakery case on the sales floor.
An internal thermometer was not present in the 2-door meat refrigerator.
Ice build up was present in the white freezer. Ice build up was present in the meat freezer. Ice build up was present in both the prepackaged and scooped ice cream freezers.
The exterior of the white freezer was soiled.
Ice cream scoops were stored in standing water with temperatures of 65°F. Unapproved sterilite containers were used to store beef jerky in the Meat Market.
Single use cups were used to store oats and sugar in the Gazebo area.
A case of single service cups was stored on the floor in the Gazebo area.
The handsink was not securely sealed to the wall in the kitchen.
The floor was rusted in the Produce walk-in cooler. The ceiling tiles were stained above the walk-in cooler. The walls were dirty and FRP board was not secured in the hallway to the rear exit door. The wall was soiled above the large3-compartment sink.
The light was not functioning in the 2-door freezer in the kitchen. A light was uncovered in the ventilation hood. Adequate lighting (less than 50 candles), was not provided in the wait station area.
An unused soda cooler and ice cream freezer were present at the rear of the store. The food storage area was unaorganized.
