Cold water was not provided to the hand sink in the women's restroom.
The work counter was chipped by the floor mixers. Rust was observed on the racks in the 2-door refrigerator. A domestic refrigerator was present in the facility.
Rust was observed on the floor mixers. Mold was observed on the caulking at the 3-compartment sink. The 2 burner stove was not installed within the ventilation hood. The lid was cracked on the raised flour storage bin below the work counter by the floor mixers.
The flour scale was soiled with flour residue. The interior of the 2-door refrigerator was soiled. Ice build up was observed in the white freezer.
The interior of the donut glazing counter cabinet was was soiled with food debris. The exterior of the storage bins were soiled. The exterior of various equipment was soiled with food debris.
A straw steamer was present on the shelving by the back door. Soiled tongs and a strainer were stored in a soiled card board box by the fryer. A metal bowl and 2 food container lids were present on the floor beside the 3-compartment sink. Domestic dish ware was present on the shelf below the 3-comaprmtnet sink.
Mold was present on the caulking of the men's restroom hand sink.
A liner was not provide for the trash can.
The cinder block wall was in disrepair and dry wall was on the floor in the mop sink room due to an auto accident. The base coving was not secured to the wall below the 2 burner stove.
A childs play pen was present in the kitchen.
The urinal was out of order in the men's restroom.
