Several bottles of liquor in the bar area were adulterated with fruit flies.
An employee was observed putting on gloves without first washing hands in the preparation area.
An employee used the hand sink next to the kitchen entrance to fill a cup of water.
The final sanitization rinse of the dish machine was less than 50 ppm chlorine residual.
A shut-off was present downstream of the atmospheric vacuum breaker on the mop sink (y-valve with shut offs).
No paper towels were available at the hand sink in the bar area.
The pop dispenser filter was soiled.
The caulking on the hand sink by the kitchen entrance is in poor repair.
The basin of the floor behind the preparation sink was soiled. The wall behind the cold-top was in poor repair.
The lights under the vent hood were not working. A light was not working in the mens restroom was not working. Three lights were out in the womens restroom.
The vent fan cover in the womens and mens restroom was soiled with dust.
