An employee was observed donning gloves without first washing their hands.
An employee was observed washing dishes in the preparation sink.
Employee food was observed next to customer food in the reach-in refrigerator.
An employee was observed handling ready-to-eat bread with their bare hands.
An employee stated that soup was reheated by being placed in the steam table.
Tuna, sliced swiss cheese, and sliced chicken were above 41�°F in the cold-top across from the bread oven. See voluntary condemnation form.

The floor beneath the back preparation tables and the hallway with the reach-in refrigerators was cracked and in poor repair. Ceiling tiles were misplaced above the reach-in refrigerator.
