A moldy lemon was stored with wholesome lemons in the walk in produce cooler. Two bottles of liquor in the bar area were adulterated with fruit flies.
A consumer advisory disclosing the increased risk of food borne illness due to consumption of raw or undercooked foods of animal origin was not provided on the menu.
Employee beverages were stored on the shelf above the preparation sink on the north side of the preparation room.
Shredded lettuce was 45�°F in the cold-top reach in refrigerator on the east end of the cooks line. Sesame chicken was 52�°F in the cold drawer across from the fryer. Tofu was 53�°F in the ice bath next to the soup wells on the wait station line.
The hand sink across from the sushi area was leaking from the drainpipe into a bucket and onto the floor.
