PROVIDE SOAP AND PAPER TOWELS AT ALL HAND SINKS IN KITCHEN AND WASHROOM AREAS. All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines. PROVIDE PEST CONTROL LOG BOOK. LOG BOOK MUST INCLUDE COPY OF PEST CONTROL LICENSE, MSDS SHEETS OF CHEMICALS USED AND MAP OF TREATED AREAS. A certified food service manager must be present in all establishments at which potentially hazardous food is prepared or served. NO FOOD SANITATION CERTIFICATE. MUST PROVIDE ORIGINAL CHICAGO FOOD SANITATION CERTIFICATE OR PROOF OF ENROLLMENT IN COURSE. All dishwashing machines must be of a type that complies with all requirements of the plumbing section of the Municipal Code of Chicago and Rules and Regulation of the Board of Health. PROPERLY INSTALL 3-COMP SINK WITH METAL DRAINBOARD AND GREASE INTERCEPTOR. The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. REPAIR FLOORS IN CLASSROOM ROOM AREAS. FLOORS NEED CLEANING UNDER ALL EQUIPMENT ALONG WALLS AND IN ALL CORNERS IN ALL AREAS. The walls and ceilings shall be in good repair and easily cleaned. REPLACE MISSING BASEBOARDS IN ALL AREAS. WALLS NEED CLEANING AND PAINTING IN UTILITY ROOM. SEAL ALL OPENINGS ALONG BASEBOARDS, AROUND PIPE FITTINGS UNDER SINKS IN WASHROOM, OUTLET AND LIGHT SWITCH COVERS IN KITCHEN, ETC. SEAL ALL RAW WOOD SURFACES (DOOR FRAME IN KITCHEN, BASEBOARDS, ETC). The flow of air discharged from kitchen fans shall always be through a duct to a point above the roofline. PROVIDE VENTILATION (HOODED AND DUCTED TO ROOF) OVER STOVE IN KITCHEN. REPLACE MISSING TANK COVER ON TOILET IN CHILDREN WASHROOM AND REPAIR TOILETS. All parts of the food establishment and all parts of the property used in connection with the operation of the establishment shall be kept neat and clean and should not produce any offensive odors. REMOVE CLUTTER IN CHILDREN WASHROOM. ORGANIZE ALL OFFICE, BASEMENT, AND STORAGE AREAS. STORE MOP HEADS UPRIGHT TO PREVENT INSECT BREEDING.
