NOTED PINK AND BLACK SLIMY SUBSTANCE INSIDE (BEHIND UPPER PANEL) WHERE THE WATER FLOWS. INSTRUCTED TO WASH, RINSE AND SANITIZE THE AFFECTED AREAS. SERIOUS CITATION 7-38-005 (A)
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. INSTRUCTED TO CLEAN AND MAINTAIN FLOORS ALONG THE WALLS AND IN ALL CORNERS THROUGHOUT REAR STORAGE AREA, KITCHEN AREA ESPECIALLY UNDER AND AROUND COOKING EQUIPMENT, COOLERS AND FREEZERS AND FLOORS THROUGHOUT BAR AREA.--------NOTED CERAMIC FLOOR TILES IN BAR AREA IN POOR REPAIR (CRACKED AND MISSING). INSTRUCTED TO FIX.
The walls and ceilings shall be in good repair and easily cleaned. INSTRUCTED TO REPLACE SOILED WATER STAINED CEILING TILES ABOVE BAR AREA.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. INSTRUCTED TO REPAIR STRIPPED COLD FAUCET HANDLE AT 3 COMPARTMENT SINK IN THE KITCHEN.--------MUST PROVIDE 3 WORKING DRAIN STOPPERS AT 3 COMPARTMENT SINK IN THE KITCHEN.
All food establishments that display, prepare, or store potentially hazardous foods shall have calibrated metal stem thermometers, provided and conspicuous, for refrigerated and hot food units. INSTRUCTED TO PROVIDE THERMOMETER VISIBLE INSIDE ALL COOLERS AND FREEZERS.
All parts of the food establishment and all parts of the property used in connection with the operation of the establishment shall be kept neat and clean and should not produce any offensive odors. INSTRUCTED TO REMOVE ALL UNNECESSARY ARTICLES STORED ON TOP OF REAR WASHROOM IN REAR STORAGE AREA AND IN GARAGE AREA TO PREVENT RODENT AND INSECT HARBORAGE.
Shielding to protect against broken glass falling into food shall be provided for all artificial lighting sources in preparation, service, and display facilities. LIGHTS MUST BE SHIELDED IN HOOD ABOVE COOKING EQUIPMENT.
All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. INSTRUCTED TO CLEAN AND MAINTAIN INTERIOR AND EXTERIOR SURFACES OF ALL COOLERS AND FREEZERS WITH DRIED FOOD DEBRIS ACCUMULATION.
