Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. FOUND HOSE FROM DISH DRAIN RACK DRAINING INTO MIDDLE COMPARTMENT OF THE 3 COMPARTMENT SINK. INSTRUCTED TO RELOCATE THE SAID HOSE PROPERLY INTO FLOOR DRAIN.
All parts of the food establishment and all parts of the property used in connection with the operation of the establishment shall be kept neat and clean and should not produce any offensive odors. NOTED CLUTTER AND UNUSED EQUIPMENT STORED ON SHELF IN PREP AREA AND IN HALLWAY LEADING TO KITCHEN. INSTRUCTED TO REORGANIZE, DISCARD ALL UNNECESSARY ARTICLES TO PREVENT RODENT AND INSECT HARBORAGE.
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. NOTED HAND SINK CABINET IN MEN'S WASHROOM IN POOR REPAIR. INSTRUCTED TO FIX OR REPLACE.

Between uses and during storage ice dispensing utensils and ice receptacles shall be stored in a way that protects them from contamination. ICE SCOOP MUST BE STORED IN A SEPARATE CLEAN CONTAINER TO PREVENT CONTAMINATION.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. INSTRUCTED TO CLEAN THE FLOORS ALONG THE WALLS AND IN ALL CORNERS IN PREP AREA AND IN BOILER/MOP ROOM.
