All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines. OBSERVED 45 DRAIN FLIES THROUGHOUT BAR AND BASEMENT AREA AT ICE MACHINE. ALSO OVER 100 SMALL ANTS ON SHELVING THROUGHOUT BAR, AT BAR THREE COMPARTMENT SINK AND ON JUICE MACHINE. INSTRUCTED MUST HAVE PEST CO. TREAT FOR BOTH, CLEAN ALL AFFECTED AREAS AND REMOVE MOIST ORGANIC MATERIAL AND POOLING WATER THROUGHOUT BASEMENT.
All dish washing machines must be of a type that complies with all requirements of the plumbing section of the Municipal Code of Chicago and Rules and Regulation of the Board of Health. WASTE WATER BACKING UP INTO TWO SINK COMPARTMENTS WHEN WATER DRAINS INTO THIRD AT KITCHEN THREE COMPARTMENT SINK. INSTRUCTED TO REPAIR TO PROPERLY DRAIN IN ALL THREE SINKS AND USE BAR THREE COMPARTMENT SINK TO WASH, RINSE AND SANITIZE.
A separate and distinct offense shall be deemed to have been committed for each Minor violation that is not corrected upon re-inspection by the health authority. PREVIOUS MINOR VIOLATIONS FROM 2-5-09 #154913 NOT CORRECTED. #33-MUST DETAIL CLEAN THE FOLLOWING: FAN COVERS IN THE WALK IN COOLER, FOOD DEBRIS ON TRAYS INSIDE PIZZA OVENS, SLIME BUILD UP ON SODA GUNS AND HOLDERS. #34-MUST CLEAN, REMOVE GREASE AND DEBRIS BEHIND COOKING EQUIPMENT. #35-DETAIL CLEAN THE WALLS AND CEILING IN WALK IN COOLER. #38-THE BAR THREE COMPARTMENT SINK HAS RUBBER GASKETS AROUND THE DRAINS WHICH PREVENTS ALL THE WASTE WATER FROM PROPERLY DRAINING. MUST REPAIR SO ALL WATER DRAINS FROM SINKS. DRAIN LINE LEAKING FROM HVAC ONTO BASEMENT FLOOR, MUST REPAIR.
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. MUST NOT USE TIN FOIL AS LINER FOR SHELVING IN KITCHEN.

OBSERVED APPROXIMATELY 50 RAT DROPPINGS ON THE FLOOR OF THE PREP AREA UNDER THE 3 COMPARTMENT SINK AND PREP COOLER, INSIDE CABINETS BEHIND THE BAR, BEHIND PIZZA STATION COOLER, ON THE FLOOR OF THE BASEMENT AND SHELVES IN THE BASEMENT. MUST CLEAN AND SANITIZE THE AFFECTED AREAS. ALSO, OBSERVED APPROXIMATELY 20 LIVE FRUIT FLIES ON THE WALLS OF THE RESTROOMS, PREP AREA AND BEHIND THE BAR. ADDITIONAL PEST CONTROL SERVICE IS NEEDED TO MINIMIZE OR ELIMINATE THE PEST ACTIVITY. SERIOUS VIOLATION 7-38-020.

MANAGEMENT INSTRUCTED TO USE THE ICE FOR ONLY ONE PURPOSE, NOT FOR COOLING BOTTLES AND TO SERVE IN DRINKS. MANAGEMENT DISCARDED THE ICE IN THE BINS DURING THE INSPECTION. SERIOUS VIOLATION 7-38-005A.
