The area outside of the establishment used for the storage of garbage shall be clean at all times and shall not constitute a nuisance. OUTSIDE GARBAGE AREA AND GARBAGE CORRAL WITH CARDBOARD BOXES ON GROUNDS, CARDBOARD FOOD CONTAINERS AND AN EXCESSIVE AMOUNT OF DEAD LEAFS ON GROUND SURROUNDING WASTE CONTAINERS. FOOD WASTE CONTAINER FOUND WITH OPEN LIDS AND FOOD DEBRIS INSIDE. INSTRUCTED TO REMOVE ALL DEBRIS FROM GROUND AND LIDS MUST BE CLOSED AND TIGHT FITTING AT ALL TIMES. RECOMMEND USING CHAINS THAT WERE PROVIDE TO SECURE LIDS.
A separate and distinct offense shall be deemed to have been committed for each Minor violation that is not corrected upon re-inspection by the health authority. PREVIOUS VIOLATIONS FROM 11-19-07 #134967 NOT CORRECTED. #33-MUST CLEAN AND MAINTAIN INTERIOR SURFACES OF TWO COMPARTMENT SINK. #41-MUST CLEAN AND REMOVE ALL UNNECCESSARY ARTICLES IN REAR STORAGE ROOM/AREAS TO PREVENT RODENT HARBORAGE. ALL ARTICLES MUST BE ELEVATED 6' FROM FLOORS FOR EASY ACCESS FOR CLEANING.
Shielding to protect against broken glass falling into food shall be provided for all artificial lighting sources in preparation, service, and display facilities. MUST PROVIDE LIGHT SHIELDS ABOVE PRODUCE AREA.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. FLOOR OF ICE CREAM FREEZER WITH PAPER DEBRIS AND EXCESSIVE ICE BUILD-UP. MUST REMOVE AND MAINTAIN.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. UTILITY MOP SINK EXTREMELY DIRTY. MUST CLEAN AND MAINTAIN.
