All food not stored in the original container shall be stored in properly labeled containers. MUST LABEL PREP FOODS INSIDE STORAGE COOLERS.
Must remove unnecessary articles thru-out basement, clean and organize to prevent pest harborage.
Between uses and during storage ice dispensing utensils and ice receptacles shall be stored in a way that protects them from contamination.
Source of cross contamination inside ice machine. Areas of black, slimey mold-like deposits in contact with ice and ice contact surfaces. Condensation from slimey areas dripping onto ice used for consumption. Critical violation. 7-38-005a. Instructed to discard ice and have machine cleaned and sanitized. Must obtain bagged ice from an approved vendor. Tag held for inspection placed on ice machine, when ready fax letter requesting tag removal.
Exposed handsink that was in basement food prep area has been removed, only water supply lines and drain pipe remain. Found knife, prep table and slicer with food debris on them. Also, large mixer/grinder in basement. Critical violation. 7-38-030. Must properly re-install exposed handsink or remove all food preparation equipment and utensils from basement.
Outermost front door not sealed to be insect/rodent proof, noted 1/4 opening to outside at bottom of door. Must caulk or seal opening around conduit that enters building from outside patio wall. Sighted live small flies in bar area and basement prep/storage areas. Also, live ants on floor in kitchen behind freezer. Serious violation. 7-38-020. Must repair/seal front door, eliminate insect activity. Recommended additional service by pest control company.
Outside waste grease container and garbage area not maintained. Sighted accumulated grease on edges of waste grease container. Over 40 milkcrates stacked outside rear of establishment, outside open linen container has litter piled inside of it. Serious violation. 7-38-020. Must clean and maintain outside areas to prevent pest harborage.

OBSERVED DISH WASHING AREA 3PART PIPES LEAKING, MUST REPAIR OR REPLACE AND MAINTAIN.
The walls and ceilings shall be in good repair and easily cleaned. MUST REPAINT CEILING IN KITCHEN AND DISH WASHING AREA. ALSO CLEAN WALL BEHIND EQUIPMENT, UNDER SERVING COUNTER AT BAR AREA.
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. MUST INSTALL DRAIN LINE FOR ONE OF THE SODA GUN HOLDER AT BAR AREA AND MAINTAIN.
All utensils shall be thoroughly cleaned and sanitized after each usage. THE FOLLOWING NOT CLEAN, MUST CLEAN/SANITIZE AND MAINTAIN, COOLERS AT BAR AREA AND KITCHEN, CHEST FREEZERS, COOKING EQUIPMENT AND SLICERS AT BASEMENT PREP AREA.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. MUST DATAIL CLEAN FLOORS, THRU-OUT PREMISES INCLUDING BASEMENT, UNDER, BEHIND EQUIPMENT AT CORNERS AND ALONG WALLS. ALSO MUST SEAL RUSTY FLOOR INSIDE WALK IN COOLER IN BASEMENT.
