Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. Found p-trap at exposed hand sink at rear of premises leaking.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. Found floors not clean at 2nd floor storage areas.
The walls and ceilings shall be in good repair and easily cleaned. Found ceiling tiles in poor repair at rear storage areas. (must replace worn ceiling tiles)
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repa. Found tables, counters, and exterior of ice machine not wiped down.

All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. THE FOLLOWING NOT CLEAN- INTERIOR CABINET OF FRONT PREP 3-COMPARTMENT SINK WITH STAINS/DEBRIS BUILD UP, PREP FRONT PREP AREA STAND UP REACH IN COOLER FAN COVER WITH ACCUMULATED DUST BUILD UP. MUST CLEAN AND MAINTAIN UNCLEAN EQUIPMENT.

MUST REPAIR FLOOR, FLOOR MUST BE SMOOTH AND EASY TO CLEAN AND KEEP DRY ALSO MUST CLEAN AND MAINTAIN INTERIOR OF ALL UNCLEAN FLOOR DRAINS.

All rooms in which food or drink is prepared, or in which utensils are washed, shall be lighted so that a minimum of 50-foot candles of light is available on all work surfaces. WALK IN FREEZER #7 LIGHT BULB BURNED OUT. MUST PROVIDE WORKING LIGHT BULB FOR FREEZER.
