The licensee shall inform the public in writing of the Chicago Department of Public Health?s Consumer Advisory relating to consuming raw and undercooked animal products. LABEL THE FOOD CONTAINERS FLOUR, SUGAR ECT.
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. THE HOOD OVER THE PIZZA OVEN HAS PEELING PAINT. INSTRUCTED TO REMOVE THE PEELING PAINT. THE TWO FREEZERS IN THE BASEMENT THICK BUILDUP AND GASKETS AND TOPS IN POOR REPAIR. INSTRUCTED TO REPLACE THEM SO TOPS WILL CLOSE PROPERLY OR GET NEW FREEZERS.
All utensils shall be thoroughly cleaned and sanitized after each usage. CLEAN THE COOKING EQUIPMENT THICK WITH GREASE BUILDUP.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. CLEAN THE FLOORS IN THE BASEMENT ALONG THE WALLS AND IN THE CORNERS HAS DUST AND DIRT ON THEM.
The walls and ceilings shall be in good repair and easily cleaned. THE CEILING TILES ARE STAINED IN THE PREP AREA. INSTRUCTED TO REPLACE OR PAINT THEM.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. CLEAN ALL THE VENTS IN THROUGHOUT PREMISES.

DIRTY SPACES WITH MISSING WALL TILES ABOVE THE EXPOSED HANDSINK AND GRILL PREP AREA FOUND, CLEAN AND REPAIR AREAS SO THEY CAN BE EASILY CLEANED. The walls and ceilings shall be in good repair and easily cleaned.
THE BASEMENT COOLER HAS NO SHIELD ON THE BULB AND THE SHIELDS ABOVE THE PIZZA PREP AREA ARE DAMAGED, REPLACE MISSING AND BROKEN SHIELDS. Shielding to protect against broken glass falling into food shall be provided for all artificial lighting sources in preparation, service, and display facilities.
