All food should be properly protected from contamination during storage, preparation, display, service, and transportation. OBSERVED MOLD GROWTH ON INTERIOR CONTACT SURFACES OF THE ICE MACHINE IN THE BASEMENT (LEFT SIDE). UNIT TAGGED BY CDPH. MANAGEMENT INSTRUCTED TO HAVE THE ICE MACHINE CLEANED AND SANITIZED. FAX REINSPECTION LETTER TO (312) 746-4240. SERIOUS VIOLATION 7-38-005A.
All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines. OBSERVED APPROXIMATELY 15-20 LIVE FRUIT FLIES BEHIND THE BAR AREAS. ADDITIONAL PEST CONTROL SERVICE IS NEEDED TO MINIMIZE OR ELIMINATE THE PEST CONTROL ACTIVITY. SERIOUS VIOLATION 7-38-020.
OBSERVED WASTE WATER BACKUP FROM THE FLOOR DRAIN BEHIND THE 1ST FLOOR AND THE BASEMENT BARS. THESE BARS CANNOT BE USED TO WASH, RINSE AND SANITIZE GLASSES. MUST REPAIR THE FLOOR DRAINS SO THAT THE WATER DRAINS PROPERLY. SERIOUS VIOLATION 7-38-030.
Adequate and convenient hand washing facilities shall be provided for all employees. THE HANDWASHING SINK FOR THE SOUTH 1ST FLOOR BAR IS NOT CONNECTED TO THE DRAIN LINE. The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. DETAIL CLEAN THE FLOOR UNDERNEATH ALL OF THE BARS.
SINK STOPPERS ARE NEEDED AT ALL OF THE 3 COMPARTMENT SINKS. REPAIR THE WATER LEAKS UNDER THE 4 COMPARTMENT SINK IN THE 1ST FLOOR MAIN BAR.
