All food shall be protected from contamination and the elements, and so shall all food equipment, containers, utensils, food contact surfaces and devices, and vehicles.DID OBSERVE FOOD (ICE) NOT PROTECTED FROM CONTAMINATION DURING SERVICE. OBSERVED ALL THREE ICE BIN SHOOT DISPENSERS AT SODA STATIONS WITH BLACK SLIME SUBSTANCE INSIDE ICE SHOOT THAT DISPENSES AND TOUCHES THE STORED ICE AT SELF SERVE AND CARRY OUT WINDOW. INSTRUCTED TO DISCARD ALL ICE IN BINS, CLEAN AND SANITIZE ALL BINS. BINS TAGGED HELD FOR INSPECTION AND MUST NOT USE UNTIL INSPECTED BY C.D.P.H.
The walls and ceilings shall be in good repair and easily cleaned. WALL MOUNTED PAPER TOWEL DISPENSER AT EXPOSED HAND SINK MUST NOT BE ABOVE PREP TABLE. HOLES IN CEILING AROUND METAL HOLDER FOR SODA DISPENSER CABLES AND AROUND CEILING AT DRIVE THROUGH REGISTER MUST SEAL. CAULKING ALONG WALL AT LARGER THREE COMPARTMENT SINK MISSING AND IN POOR REPAIR,ALSO AT MOP SINK WITH BLACK MOLD,MUST REPLACE.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. FLOOR DRAIN AT KITCHEN ICE MACHINE MUST CLEAN AND MAINTAIN.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. THREADED NOZZLES AT THREE COMPARTMENT SINK AND ON NOZZLE ABOVE TAYLOR ICE CREAM MACHINE MUST BE REMOVED.
All food establishments that display, prepare, or store potentially hazardous foods shall have calibrated metal stem thermometers, provided and conspicuous, for refrigerated and hot food units. MUST PROVIDE INTERNAL THERMOMETERS FOR COOKS LINE CHEESE COOLER AND WALK IN COOLER.

OBSERVED APPROXIMATELY 1 LB COOKED BACON @79F HELD IN CONTAINERS ON SHELVING UNDER PREP TABLE. MANAGEMENT STATES THOSE FOODS ARE TO BE HELD AT ROOM TEMPERATURE ACCORDING TO BURGER KING POLICY. ALL FOODS DISCARDED.

THE FAN MOTOR WAS REPLACED BY THE TECHNICIAN DURING THE INSPECTION. COOLER WAS REPAIRED DURING THE INSPECTION AND THE AIR TEMPERATURE WAS AT 39.9F. CRITICAL VIOLATION 7-38-005A.
OBSERVED POTENTIALLY HAZARDOUS FOODS AT IMPROPER TEMPERATURES. OBSERVED WATER LEAKING FROM THE CEILING TILES IN THE REAR AREA NEAR THE BACK DOOR AND IN THE DINING ROOM AREA OVER ONE OF THE TABLES. MUST MAKE REPAIRS TO THE ROOF TO PREVENT LEAKING. DETAIL CLEAN THE CEILING TILES ABOVE THE BOXES OF FOUNTAIN POP LOCATED IN THE CORNER NEAR THE REAR EXIT DOOR. DETAIL CLEAN THE WALLS IN THE PREP AREA.
MUST PROVIDE THERMOMETERS INSIDE OF ALL REFRIGERATION UNITS INCLUDING PREP COOLERS.
