A separate and distinct offense shall be deemed to have been committed for each Serious violation that is not corrected upon re-inspection by the health authority. No City of Chicago food service sanitation managers present during the preparation of potentially hazardous foods. Report #157842 dated June 2, 2009 Also documented no certified manager. Obtain proof of enough City of Chicago food service sanitation managers to cover your hours of operation.
All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines. A dozen live smallflies were found in the dishroom and seem to be coming from the improperly trapped mop sink. Remove all evidence of smallflies and have the pest control company update the log book licenses.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. Clean and maintain the floors and baseboards so they are smooth and sanitary from corner to corner.

THE ICE MACHINE HAS SOME DIRTY BUILDUP INSIDE AND NEEDS TO BE WASHED, RINSED AND SANITIZED. DRIED UP MILK SPILLS FOUND IN ALL THREE OF THE TWO DOOR COOLERS NEED TO BE CLEANED, RINSED AND SANITIZED.
AN ABUNDANCE OF DUST AND COBWEBS BEHIND THE BARISTA STATION COOLER NEEDS TO BE CLEANED.

NO CITY CERTIFIED MANAGER OR PROOF OF ON DUTY WHILE POTENTIALLY HAZARDOUS FOODS ARE BEING PREPARE AND SERVED, SUCH AS HOT SANDWICHES AND MILK DRINKS. CITATION ISSUED 7-38-012.
