May 7, 2012OBSERVED APPROXIMATELY 20 RODENT MICE DROPPINGS IN VARIOUS AREAS OF THE DAY CARE INSIDE THE MOP SINK ROOM,ELECTRICAL ROOM,UTILTY CLOSET LOCATED IN THE INFANT ROOM. ALSO,1 LIVE ANT ALONG THE WALL BASE COVING IN THE EAST WALL IN TODDLER ROOM.ADDITIONAL PEST CONTROL SERVICE IS NEEDED TO MINIMIZE OR ELIMINATE THE PEST ACTIVITY.THE FRONT DOORS DIRECTLY TO THE DAY CARE CENTER THERE IS A 1/2' GAP ALONG THE BOTTOM OF THE DOORS. DETAIL CLEAN,SANITIZE ALL AFFECTED AREAS,RODENT PROOF SAID DOORS.SERIOUS VIOLATION 7-38-020.
