All employees shall be required to use effective hair restraints to confine hair. HAIR RESTRAINTS ARE NEEDED FOR ALL FOOD HANDLING EMPLOYEES.
A certified food service manager must be present in all establishments at which potentially hazardous food is prepared or served. NO CERTIFIED FOOD MANAGER WITH A CITY OF CHICAGO FOOD SANITATION CERTIFICATE WAS PRESENT WHILE POTENTIALLY HAZARDOUS FOODS WERE BEING PREPARED (FISH, VEGETABLES, ETC.). MANAGEMENT INSTRUCTED THAT A CERTIFIED FOOD MANAGER MUST BE PRESENT WITH THEIR ORIGINAL CITY OF CHICAGO FOOD SANITATION CERTIFICATE AT ALL TIMES WHILE POTENTIALLY HAZARDOUS FOOD IS BEING PREPARED OR SERVED. SERIOUS VIOLATION 7-38-012.
One copy of the Food Inspection Report Summary must be displayed and visible to all customers. NO INSPECTION REPORT SUMMARY WAS POSTED. THE PREVIOUS INSPECTION REPORT SUMMARY FROM 4/9/09 WAS STORED IN A FOLDER IN THE OFFICE. MANAGEMENT WAS GIVEN A COPY OF TODAY'S INSPECTION SUMMARY REPORT AND WAS INSTRUCTED TO POST IT IN PLAIN VIEW OF THE ESTABLISHMENT'S CUSTOMERS. SERIOUS VIOLATION 7-42-010B.
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. OBSERVED EXPOSED INSULATION INSIDE OF THE MEAT COOLER DOORS DUE TO BROKEN PLASTIC. MUST REPAIR BOTH DOORS INSIDE OF THE MEAT COOLER BELOW THE PREP COUNTER.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. MUST REGROUT THE FLOOR TILES IN THE POTWASH AREA AND AROUND THE HIGH TEMPERATURE DISHMACHINES. DETAIL CLEAN THE FLOOR BEHIND THE OVENS.
OBSERVED THE BASEMENT MOP SINK BACKING UP WITH WATER. MUST REPAIR THE SINK SO THAT IT DRAINS.
