All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines. found observed rear exit door in bsmt, front entrance doors not rodent, doors had gaps ranging from 1/4 inch to 1/2 inch.citation issued H000062803 with a court date of 8/05/2010, at 400 w superior in room 112 at 9:00 am.
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. caulk in 1st bathroom sink(south quadrant)in state of disrepair, shall be repair/replaced. non food contact surfaces of cabinet shelving in prep area in state of disrepair, shall be repair/replaced.

OBSERVED HIGH TEMP DISH MACHINE HOSE ATTACHED AT WATER PIPE UNDER THE 2 COMPARTMENT SINKS,INSTRUCTED TO PROVIDE A BACK FLOW PREVENTER AT SAID UNIT.
ALL ITEMS STORED IN BASEMENT(LAUNDRY ROOM/HOT WATER TANK ROOM) MUST BE ELEVATED. REMOVE WHAT'S NOT NEEDED AND STORE ITEMS NEEDED 6' OFF THE FLOOR.

REMOVE SPIDER WEB BY THE EAST WINDOW BETWEEN WALL AND BOOKCASE, NORTH BUILDING.
ELEVATE DIAPERS 6' FROM FLOOR INSIDE THE STORAGE CLOSET,AT EAST PLAY ROOM,CLEAN DEBRIS ON SAID CLOSET.
PAINT WALL UNDER THE TWO COMPARTMENT SINK, IN KITCHEN. SEAL HOLE AROUND WATER PIPE UNDER THE CABINET OF WASH BOWL IN PLAY ROOM.
OBSERVED WINDOW OPENED ABOVE THE TWO COMPARTMENT SINKS,INSTRUCTED TO PROVIDE SCREEN. ALSO REPLACE BROKEN SCREEN AT WINDOW AT OFFICE REAR AREA AT SOUTH BUILDING.
