The walls and ceilings shall be in good repair and easily cleaned. NOTED BLACK MOLD LIKE SUBSTANCE ON BASEMENT SOUTH WALL (UNPAINTED DRYWALL) AND ON WALL BY STAIRS TO BASEMENT (ALSO UNPAINTED DRYWALL). ALSO NOTED MOLD LIKE ODOR IN SAME AREA. INSTRUCTED MANAGER TO HAVE BLACK SUBSTANCE REMOVED OR REPLACE WALL. ALL WALLS MUST BE PROPERLY SEALED. FOOD DEBRIS ON WALL BEHIND DISH MACHINE. MUST DETAIL CLEAN WALLS HERE AND THROUGHOUT PREMISES AS NEEDED. INADEQUATE SHELIVING. MUST PROVIDE SHELVES THROUGHOUT PREMISES AND STORE ALL ITEMS 6' OFF FLOOR. REPLACE MISSING CEILING TILE IN REAR DISHWASHING AREA.
Shielding to protect against broken glass falling into food shall be provided for all artificial lighting sources in preparation, service, and display facilities. MUST PROVIDE MISSING LIGHT SHIELDS IN FRONT SERVICE AREA.
All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. DETAIL CLEAN ALL EQUIPMENT INCLUDING FRONT SERVICE UNITS AND CABINETS, MILK FROTHER, REFRIGERATION UNITS ETC THROUGHOUT PREMISES TO REMOVE FOOD DEBRIS. HAVE INTERIOR OF ICE MACHINE DETAIL CLEANED.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. DETAIL CLEAN FLOORS IN FRONT SERVICE AREA NEXT TO MILK STATION AND THROUGHOUT PREMISES AS NEEDED TO REMOVE ACCUMULATED GRIME.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. CLEAN DUST FROM VENT COVERS IN WASHROOM AND REFRIGERATION UNITS. REPLACE MISSING SEWER CAP, REPAIR BROKEN CEMENT AROUND SEWER OUTSIDE REAR DOOR IN CO0MMON AREA.

OBSERVED EVIDENCE OF MICE DROPPINGS IN DIFFERENT LOCATIONS OF PREMISES,FRONT AREA:UNDER SHELVES AND BEHIND PIPE,EAST SIDE OF THE COUNTER;REAR AREA BETWEEN THE ELECTRICAL HOT WATER TANK AND DISH MACHINE,UNDER NORTH SHELF BY THE DESK,UNDER THE DESK AND ON TOP WHITE ELECTRICAL BOX; BEHIND THE DOOR OF STAIRS THAT GOES DOWN TO BASEMENT.
