All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. FOUND MANAGEMENT REUSING TIN CANS FOR FOOD STORAGE (EG SOY SAUCE, EGGS, SAUCE,CUT VEGETABLES ETC.) INSTRUCTED TO USE FOOD GRADE CONTAINERS.
All parts of the food establishment and all parts of the property used in connection with the operation of the establishment shall be kept neat and clean and should not produce any offensive odors. REAR STORAGE BY STAIRCASE OUTSIDE AREA(BEHIND REAR DOOR) WITH CLUTTER AND UNNECESSARY ARTICLES. INSTRUCTED TO CLEAN AND MAINTAIN. ----- INSTRUCTED TO REMOVE ALL UNNECESSARY ARTICLES AND UNUSED EQUIPMENT (EG PLASTIC BAGS, PAPER, BROKEN MICROWAVE OVEN ETC.) STORED THROUGHOUT PREMISES (FRONT AND REAR STORAGE AREA) TO PREVENT RODENT AND INSECT HARBORAGE. -----INSTRUCTED TO REORGANIZE STORAGE SHELVES IN REAR STORAGE AREA AND BEHIND FRONT COUNTER SHELVES. ITEMS MUST BE STORED 6'OFF THE FLOOR FOR EASY ACCESS ON CLEANING.
All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. INSTRUCTED TO CLEAN AND MAINTAIN THE FOLLOWING: INTERIOR AND EXTERIOR SURFACES OF ALL COOLERS WITH DRIED FOOD DEBRIS AND SPILLS, INTERIOR AND EXTERIOR SURFACES OF STOVE ENCRUSTED WITH GREASE AND DRIED FOOD, EXTERIOR SURFACES OF GREASE BOX, INTERIOR OF ALL STORAGE CABINETS THROUGHOUT PREMISES INCLUDING WASHBOWL CABINET INSIDE WASHROOM.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. INSTRUCTED TO PROVIDE 3 WORKING DRAIN STOPPERS AT 3 COMPARTMENT SINK.

INSTRUCTED TO CLEAN AND RECAULK WORN-OUT CAULKING AROUND 3 COMPARTMENT SINK.
ICE SCOOP MUST BE STORED IN A CLEAN SEPARATE CONTAINER TO PREVENT CONTAMINATION.
INSTRUCTED TO PROVIDE VENTILATION INSIDE TOILET ROOM.

Foods inside coolers not labeled or dated. Instructed to label and date all foods not in original container.
Prep table, sink, cooking equipment, coolers and can opener not cleaned. Instructed to detail clean and sanitize.
Floor along wallbase and behind equipment not cleaned. Instructed to detail clean daily.
Unnecessary articles stored in rear not cleaned. Instructed to remove all unused articles and elevate supplies 6 inches or higher.
