All food not stored in the original container shall be stored in properly labeled containers. PREPARED AND BULK FOODS.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. FLOORS IN THE FOLLOWING AREAS WITH CRACKED/MISSING TILES, THROUGHOUT FRONT BAR, AT THRESHOLD IN MENS ROOM FRONT BAR, AT REAR EXIT DOOR FRONT BAR. MUST REMOVE POOLING WATER FROM FLOOR IN BASEMENT UNDER STORED GLASSWARE/TO GO CONTAINERS.
The walls and ceilings shall be in good repair and easily cleaned. CEILING IN FRONT BAR LADIES WASHROOM WITH HOLE, MUST SEAL/REPAIR. WALL AT FOOD WARMER WINDOW WITH CHIPPING PLASTER, MUST REPAIR.
Shielding to protect against broken glass falling into food shall be provided for all artificial lighting sources in preparation, service, and display facilities. MUST PROVIDE AT BASEMENT ICE MACHINE AND KITCHEN, INCLUDING END CAPS.
Toilet rooms shall be completely enclosed and shall be vented to the outside air or mechanically ventilated. MUST REPAIR/REPLACE BROKEN SELF-CLOSING DEVICE ON REAR BAR RIGHT SIDE WASHROOM.
All food establishments that display, prepare, or store potentially hazardous foods shall have calibrated metal stem thermometers, provided and conspicuous, for refrigerated and hot food units. FOR ALL UNITS.
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. KITCHEN TWO DOOR COOLER WITH BROKEN PLASTIC DOOR GASKET, MUST REPLACE.

MUST TURN OFF MACHINE, REMOVE ICE AND HAVE THE INTERIOR OF THE ICE MACHINE CLEANED AND SANITIZED. SERIOUS VIOLATION 7-38-005A.
