NO FOOD CERTIFIED MANAGER ON DUTY WITH A CITY OF CHICAGO SANITATION CERTIFICATE, WHILE POTENTIALLY HAZARDOUS FOODS ARE COOKED AND SERVED, AT THIS TIME OF INSPECTION, INSTRUCTED TO ENROLL IN A CLASS AND A CERTIFIED MANAGER MUST BE ON DUTY AT ALL TIMES FOODS ARE COOKED AND PREPARED SERIOUS CITATION ISSUED
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. MUST RESTRICT USING UNAPPROVED FOOD CONTAINERS, FOR FOOD PREP AND STORAGE MUST PROVIDE DOORS AT FOOD DISPLAY FRONT COUNTER AREA MUST PROVIDE A SPLASH GUARD BETWEEN EXPOSED HANDBOWL AND 3 COMPARTMENT
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. MUST CLEAN FLOORS UNDER AND BEHIND EQUIPMENT 1ST FLOOR AND BASEMENT AREAS

Outer openings not protected or rodent proofed. Rear exit door and frame has been removed, only screen door in place. Rear has 1/4 to 1/2 inch gap opening around frame and screen door. Instructed to rodent proof door and install rear door to prevent rodent and insect entry. No citation issued.
Facilities not maintaining proper temperature. Two coolers not maintaining proper temperature. Internal temperature at 47.8 F for rear cooler and 58.6 F for front cooler. Instructed to repair cooler to maintain 40 F or below. No citation issued.
Potentially hazardous foods does not meet temperature requirement during storage, preparation, display and service. Found 5 lbs of Cannoli's at 57.3 F, 28 lbs of Creamed pastries at 57.8F, 8 lbs of Tiramisu at 52.4 F, 10 lbs Mozzarella Cheese at 47.4 F, Marinara Sauce 6 lbs at 47.3 F. Instructed manager that potentially hazardous foods must maintain 40 F or below. Manager voluntarily denatured and discarded. Value $170, 57 lbs No citation issued.
Ventilation not working in toilet room. Instructed to repair exhaust fan. Sprayer at three compartment not properly installed. Instructed have sprayer repair and elevate above compartment sink water level. Commercial stove in basement connected to gas line not properly vented. Instructed to install exhaust hood or remove stove from premises.
Light covers through-out prep area not cleaned. Instructed to detail clean.
