The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. OBSERVED STAINED CARPETING THROUGHOUT FRONT DISPLAY AREA AND IN REAR STORAGE AREA. INSTRUCTED TO CLEAN OR REMOVE.----- NOTE PLYWOOD FLOORS IN THE MIDDLE OF DISPLAY AREA. INSTRUCTED TO FIX OR REPLACE FLOOR TILES.
The walls and ceilings shall be in good repair and easily cleaned. INSTRUCTED TO REPLACE STAINED AND MISSING TILES THROUGHOUT PREMISES (FRONT & REAR).-----MUST CLEAN OR REPAINT WALLS THROUGHOUT REAR STORAGE AREA.--- NOTES CONCRETE PATCHED ON WALL BASEBOARD IN DISPLAY AREA. INSTRUCTED TO FIX, MUST BE SMOOTH AND EASILY CLEANABLE.
INSTRUCTED TO INSTALL SELF-CLOSING DEVICE AT WASHROOM DOOR.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. INSTRUCTED TO INSTALL MOP SINK WITH HOT AND COLD RUNNING WATER WITH BACK FLOW PREVENTER DEVICE. ---- MUST REPAIR HANGING VENTILATION INSIDE WASHROOM.
All parts of the food establishment and all parts of the property used in connection with the operation of the establishment shall be kept neat and clean and should not produce any offensive odors.
