Outer openings not protected or rodent proofed. Observed northwest door not rodent or insect proofed double doors has 1/4 inche gap at bottom where doors meet in middle. Instructed to rodent proof door to prevent rodent and insect entry. Must also repair and rodent/insect northeast door left side. Serous citation issued 7-38-020.
Unnecessary articles stored in boiler room and mobile classrooms. Instructed to remove all unused articles and store stock and supplies 6 inches off floor and away from the walls.
Floor in corner in kitchen, classrooms and along wallbase in boiler room not cleaned. Instructed to detail clean.

Non food contact surfaces of cooler shelving/racks not clean need detailed cleaning(crevices).
Cooler shelving/racks that had peeling paint/rust shall be repair/replaced.
Floors under heavy equipment, storage shelving/racks not clean need detailed cleaning(corners).
Walls in kitchen prtep area by 3 compartment sink had peeling paint, shall be repair/sealed. Walls, ceilings in classrooms in 2nd floor, and 1st floor that had peeling paint shall be repair/sealed. Peeling paint on west staircase (2nd floor west)on walls, ceiling, shall be repair, bathrooms that had peeling paint in 2nd floor, 1st floor, and annex buildings shall be repair/sealed. Exposed insulation in bsmt. storage areas shall be repair/sealed.
Light shields in kitchen prep area not clean need cleaning. Missing light shields in classrooms in 2nd floor, 1st floors shall be provided. Light shields in classrooms, hallways, bathrooms in 1st floor, 2nd floor(Main building), and annex buildings need detailed cleaning.

PELLING PAINT AND CRACKED WALLS BEHIND THE EXPOSDE HAND SINK IN THE PREP AREA/KITCHEN. MUST SCRAPE PEELING PAINT AND REPAIR CRACKED WALLS. WALLS BEHIND THE TOILET BOWL INSIDE THE WASHROOM IN THE MAIN OFFICE NOT IN GOOD REPAIR. MUST REPAIR AND MAINTAIN.
ONLY AUTHORIZED PERSONNEL IN THE PREP AREA/KITCHEN. OBSERVED STUDENTS AND TEACHERS COMING FROM THE HALLWAY GOING TO THE PREP AREA/ KITCHEN. TEACHERS ARE GETTING THEIR BREAKFAST AND STUDENTS ARE GIVING THE MONEY TO THE MANAGER.

No citation issued due to pre-operational inspection assignment.
Found bathrooms in new building addition 1st floor, 2nd floor with no soap, and soap dispenser units, must provide soap, and soap dispenser units to all staff bathrooms, student bathrooms. No citation issued due to pre-operational inspection assignment.
Hot temperature dish machine not reaching 180f for rinse cycle, above dish machine only reaching 119f for rinse cycle, hot temperature dish machine must reach 180f for final rinse.(no citation due to pre-operational inspection assignment)
Toilet in women's staff bathroom(new building addition) in state of disrepair, not working(no flushing)shall be repaired/replaced, also found washbowl sink in men's bathroom(new addition) not working, shall be repaired. No citation issued due to pre-operational inspection assignment.
Non food contact surfaces of cooking equipment, cooking equipment, front counter display units, cooler shelving not clean, need detailed cleaning, non food contact surfaces of prep tables, storage shelving not clean, need detailed cleaning.
Floors throughout kitchen, lunchroom, storage rooms, storage areas in new addition(1st floor)not clean, need detailed cleaning.
Reach in cooler in front line(in new building addition)leaking on bottom of unit, shall be repaired.
Missing ceiling tiles in classrooms(1st floor, 2nd floor)in new building addition, shall be provided, damaged ceiling tile in new building addition 2nd floor, shall be replaced. Classroom #202 had peeling paint on wall, and ceiling(old building), shall be repaired/sealed. damaged cove moulding in room #206, shall be repaired.
Light shields in front counter area (new building addition) not clean, need cleaning, light shields in classroom(new building addition) not clean, need detailed cleaning. Broken light Shield in 2nd floor bathroom(new building addition), shall be replaced. Missing light shields in classrooms #200, #100, in old building, shall be provided.Bathroom in main office and gym were missing light shield(olde building), shall be provided.
Exhaust fan/vents in bathrooms(new building addition) in state of disrepair(not working), shall be repaired/replaced. Exhaust vents in classroom old building need cleaning.
Must provide thermometer to all coolers in kitchen prep area(new building addition) in order to maintain proper temperatures of 40f or less.
