Ventilation not provided in employees toilet room. Instructed to repair exhaust fan or have window opened at all times. Utility sink in basement has hose attached to faucet and no backflow device installed. Instructed to install backflow device on faucet or remove hose and cut threads from faucet.
Supplies not properly stored in basement. Instructed to organize supplies and elevate 6inches off floor and away from wall.
Floor along wallbase, behind equipment and in basement not maintained. Instructed to detail clean floor and replace any missing floor tile.

Stock items must be stored six inches off of floor to prevent pest harborage.

Leaking faucet at mop sink, shall be repaired/sealed.
Found some food items not properly labeled/dated in cooler, must date and label food items in cooler.
Kitchen cabinet shelving that had exposed raw wood, shall be repaired/sealed.
Floor by mop sink area not clean, needs cleaning(detailed).Floor in storage closet under shelving need cleaning(corners).
