The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. INSTRUCTED TO DETAIL CLEAN AND MAINTAIN FLOORS ALONG THE WALLS AND IN ALL CORNERS THROUGHOUT PREP AREAS AND IN BASEMENT. (NOTED WITH DRIED FOOD DEBRIS AND GREASE ACCUMULATION).
The walls and ceilings shall be in good repair and easily cleaned. INSTRUCTED TO CLEAN AND MAINTAIN WALLS WITH BLACK MOLD LIKE BUILD-UP INSIDE PRODUCE WALK-IN COOLER IN REAR.----- NOTED GREASE AND DUST BUILD-UP AT HOOD/EXHAUST ABOVE COOKING EQUIPMENT (FRONT AND REAR KITCHEN), FOOD STAIN AT CEILING TILE ABOVE 2 COMPARTMENT SINK AND WATER STAINED CEILING TILES ABOVE ICE MACHINE/SERVICE AREA. INSTRUCTED TO CLEAN AND MAINTAIN.-------INSTRUCTED TO REMOVE GRAFFITI SIGNS (CLEAN AND MAINTAIN) ON WALLS, DOOR FRAMES AND CABINET INSIDE MEN AND WOMEN (CUSTOMER) WASHROOM.
Shielding to protect against broken glass falling into food shall be provided for all artificial lighting sources in preparation, service, and display facilities. LIGHT MUST BE SHIELDED AT HOOD/EXHAUST ABOVE COOKING EQUIPMENT IN FRONT COOKING AREA, REAR COOKING AREA AND INSIDE WALK-IN COOLER AND WALK-IN FREEZER IN REAR.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. INSTRUCTED TO REPAIR LEAKY DRAINPIPE UNDER MIDDLE COMPARTMENT OF THE 3 COMPARTMENT SINK AND LEAKY (HOT) FAUCET HANDLE AT 3 COMP. SINK. NOTED EMPLOYEE TOILET ROOM IN THE BASEMENT WITHOUT HAND SINK. INSTRUCTED TO INSTALL HAND SINK OR REMOVE THE SAID TOILET ROOM AND ALSO MUST PROVIDE VENTILATION INSIDE EMPLOYEE TOILET ROOM IN THE BASEMENT.
All parts of the food establishment and all parts of the property used in connection with the operation of the establishment shall be kept neat and clean and should not produce any offensive odors. NOTED CLUTTER AND UNUSED EQUIPMENT (COOLER, FRYER, FREEZER ETC.) STORED AT REAR OUTSIDE AREA AND IN BASEMENT. INSTRUCTED TO REMOVE, CLEAN AND REORGANIZE USABLE ITEMS 6' OFF THE FLOOR FOR EASY ACCESS ON CLEANING AND TO PREVENT RODENT AND INSECT HARBORAGE.
