The area outside of the establishment used for the storage of garbage shall be clean at all times and shall not constitute a nuisance. GARBAGE AREA NOT CLEANED.WE(THE MANAGER AND I) OBSERVED WASTE OIL CONTAINER NOT CLEANED, BUILD-UP OF WASTE GREASE ON TOP THE LID(LAYER OF GREASE),INSTRUCTED TO KEEP AREA AND CONTAINERS CLEANED DAT ALL TIMES TO MINIMIZE RODENT/INSECT ACTIVITIES. MANAGER PERSONALLY CLEANED THE WASTE OIL CONTAINER.(CORRECTED DURING INSPECTION) SERIOUS VIOLATION:7-38-020 H000073319-16
The walls and ceilings shall be in good repair and easily cleaned. REMOVE PEELING PAINT ABOVE THE PREP TABLE IN REAR PREP AREA. REMOVE DIRTY(BLACK SUBSTANCE0 CAULKING AROUND THE 3 COMPARTMENT SINK,CLEAN WALL AND RESEAL.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. REPAIR LEAKING PIPE UNDER THE 3 COMPARTMENT SINK. PROVIDE UN THREAD FAUCET FOR THE SAME SINK.THREE STOPPERS IS NEEDED AT 3 COMPARTMENT SINK.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. REPLACE MISSING BASEBOARD ON WALLS UNDER THE 3 COMPARTMENT SINK AND THROUGHOUT THE PREMISES.CLEAN FLOOR IN SMALL BASEMENT FOOD LIQUID BUILD-UP(POP CONTAINERS ARE STORED IN IT).
All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. CLEAN INTERIOR OF DEEP FRYER, BOTTOM PART(EXTREMELY WITH GREASE BUILD-UP).
Cleaning equipment shall be properly stored away from food, utensils, equipment, and clean linens. UNABLE TO CHECK GREASE TRAP, DUE TO MANAGER UNABLE TO OPEN FLOOR DOOR(DOOR IS STUCK)WHEN WATER IS RELEASED FROM 3 COMPARTMENT SINK A FOUL ODOR IS RELEASED.INSTRUCTED TO CLEAN GREASE TRAP REGULARLY AND MUST BE ABLE TO OPEN FLOOR DOOR UPON NEXT INSPECTION.

MUST REATTACH THE BASEBOARD TO THE WALL IN THE REAR PREP AREA WHERE IT IS LOOSE. REPAIR THE CEILING ABOVE THE REAR PREP AREA TABLE WHERE PAINT IS CHIPPED. MUST SEAL THE GAP BETWEEN THE WALL AND CEILING IN THE REAR PREP AREA ABOVE THE 3 COMPARTMENT SINK.
MUST PROVIDE LIGHT SHIELDS FOR THE LIGHT FIXTURES IN THE REAR PREP AREA.
