OUTSIDE PATIO AREA NOT CLEAN, OBSERVED DEBRIS AND CLUTTER.COUCH, CONSTRUCTION MATERIAL, WOOD , CERAMIC FLOOR TILES, GARBAGE ON GROUND, INSTRUCTED TO REMOVE ALL UNNECESSARY ARTICLE FROM IT. KEEP GROUND CLEAN AND ELEVATE ITEMS 6' OFF THR GROUND, TO PREVENT RODENT AND INSECTS ACTIVITIES. SERIOUS VIOLATION: 7-38-020 HOOOO60433-18
Shielding to protect against broken glass falling into food shall be provided for all artificial lighting sources in preparation, service, and display facilities. OBSERVED LIGHT SHIELD IN KITCHEN NOT CLEAN,(DUST AND DEAD INSECTS IN IT)INSTRUCTED TO CLEAN.
All parts of the food establishment and all parts of the property used in connection with the operation of the establishment shall be kept neat and clean and should not produce any offensive odors. OBSERVED BASEMENT AREA HAVING ITEMS ON FLOOR, NEXT TO LAUNDRY ROOM, AND LAUNDRY ROOM, INSTRUCTED TO ELEVATE ITEMS 6' OFF THE FLOOR.UNNECESSARY ITEMS REMOVE FROM AREA MENTIONED.
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. OBSERVED WHITE CHEST FREEZER FOOD IN IT STORED AT LAUNDRY AREA OF THE BASEMENT.INSTRUCTED MANAGER TO KEEP LOCKED THE UNIT, OR STORE IT IN PREP AREA. ADDITIONAL THRESHOLD IS NEEDED FOR REAR DOOR.

Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. MUST PROVIDE STOPPERS FOR THREE COMPARTMENT SINK.
All food shall be protected from contamination and the elements, and so shall all food equipment, containers, utensils, food contact surfaces and devices, and vehicles. OBSERVED UPON ARRIVAL FOR INSPECTION EMPLOYEE EATING IN DINING ROOM THEN ENTER FOOD PREP AREA AND PROCEED TO TAKE PREPARED FOODS FROM REACH-IN COOLER AND COVER WITH PLASTIC WRAP AND PERFORM OTHER DUTIES THROUGHOUT FOOD PREP AREA WITH OUT WASHING HANDS UPON ENTERING PREP AREA. ALSO NOTED NO SOAP AT KITCHEN PREP EXPOSED HAND SINK. REVIEWED PROPER HAND WASHING AND GOOD HYGIENIC PRACTICES. MANAGEMENT PROVIDED SOAP AT SINK.
All employees shall be required to use effective hair restraints to confine hair. ALL EMPLOYEES MUST WEAR HAT/HAIR RESTRAINTS.
The traffic of unnecessary persons through the food preparation and the utensil washing areas is prohibited. CHILD OBSERVED IN FOOD PREP AREA HANDLING FOODS.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. FLOORS THROUGHOUT ESTABLISHMENT IN ALL PREP AREAS AND UNDER FRONT SODA DISPENSING WITH DIRT, AND FOOD DEBRIS. MUST CLEAN AND MAINTAIN. MUST NOT USE CARDBOARD AS MATTING FOR FLOORS.
Toilet rooms shall be completely enclosed and shall be vented to the outside air or mechanically ventilated. SELF-CLOSING DEVICE ON MENS ROOM DOOR BROKEN. MUST REPLACE/REPAIR.
All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. INTERIOR AND EXTERIOR OF COOKS LINE COOLER WITH EXCESSIVE GREASE. DIRTY SLICER WITH ENCRUSTED FOOD. INTERIOR COOKS LINE ONE DOOR PREP COOLER WITH FOOD DEBRIS. TALL REACH-IN FREEZER WITH EXCESSIVE ICE. MUST CLEAN AND MAINTAIN ALL.

BABY STROLLER MUST NOT BE STORED IN REAR PREP. ALL UNUSED COOLERS AND PREP TABLE ON REAR PATIO MUST BE REMOVED.
All food establishments that display, prepare, or store potentially hazardous foods shall have calibrated metal stem thermometers, provided and conspicuous, for refrigerated and hot food units. MUST PROVIDE METAL STEM THERMOMETER TO CHECK FOOD TEMPERATURES.
