REPAIR LEAKING FAUCET AND PIPE UNDER 3COMP SINK.
All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. CLEAN OUTER WALLS OF PIZZA WARMER EQUIPMENT AND INSIDE PIZZA OVEN HOOD LINE WITH DUSTS,AND CHEF REACH IN COOLER DRAWER BOTTOMS WITH SPILLS.
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. MUST REPAIR SENSOR KNOB ON PAPER NAPKIN AT PREP AREA,AND DAMAGED GASKET ON PIZZA WARMER UNIT DOOR.

All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines.OBSERVED LIVE FRUIT FLIES ON THE PREMISES AND IMPROPER PEST CONTROL METHODS. APPROXIMATELY 25 LIVE FRUIT FLIES WERE OBSERVED ON THE WALLS OF THE REAR PREP AREA AND INSIDE THE LAUNDRY ROOM. ALSO OBSERVED APPROXIMATELY 20 RODENT MICE DROPPINGS IN THE FOLLOWING AREAS CLUTTERED TOOL ROOM,REAR STORAGE ROOM,LIQUOR ROOM. MANAGEMENT WAS INSTRUCTED TO DETAIL CLEAN,SANITIZE ALL AFFECTED AREAS SEAL NOTICEABLE OPENINGS,REMOVE CLUTTERED CONDITIONS. ADDITIONAL PEST CONTROL SERVICE IS NEEDED TO MINIMIZE OR ELIMINATE THE PEST ACTIVITY. SERIOUS VIOLATION 7-38-020.
All food should be properly protected from contamination during storage, preparation, display, service, and transportation.FOOD (ICE) NOT PROTECTED DURING STORAGE INSIDE OF THE ICE MACHINE. TAGGED HELD FOR INSPECTION.OBSERVED MOLD GROWTH ON INTERIOR SURFACES OF THE ICE MACHINE. MANAGEMENT INSTRUCTED TO TURN OFF MACHINE, REMOVE ICE AND HAVE INTERIOR OF ICE MACHINE CLEANED AND SANIIZED.FAX LETTER TO THE CDPH 312-746-4240 FOR TAG REMOVAL. ALSO OBSERVED SOME FOODS UNCOVERED IN THE REAR 2 DOOR COOLER STORED ON SHELF BENEATH DUSTY FAN COVERS.MUST REMOVE. SERIOUS VIOLATION 7-38-005A.
