Equipment and utensils should get proper exposure to the sanitizing solution during the rinse cycle. Bactericidal treatment shall consist of exposure of all dish and utensil surfaces to a rinse of clean water at a temperature of not less than 180F. HIGH TEMPERATURE DISHMACHINE IS NOT OPERATIONAL. MUST REPAIR. SANITIZING RINSE ABOVE 180F REQUIRED. CRITICAL VIOLATION 7-38-030.

All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. DUSTY CEILING VENTS IN BOTH WOMANS WASHROOMS ON 1ST AND 2ND FLOORS. MUST CLEAN.
Between uses and during storage ice dispensing utensils and ice receptacles shall be stored in a way that protects them from contamination. WIPING CLOTHS FOR 2ND FLOOR DINING ROOM MUST BE STORED IN A CLEAN CONTAINER WITH A SANITIZING SOLUTION.
All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. INTERIOR ICE MACHINE MUST CLEAN AND SANITIZE.
All food establishments that display, prepare, or store potentially hazardous foods shall have calibrated metal stem thermometers, provided and conspicuous, for refrigerated and hot food units. BROKEN INTERNAL THERMOMETER MUST BE REPLACED IN RIGHT SIDE TALL REACH-IN COOLER.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. LEAK UNDER DISH AREA EXPOSED HAND SINK. MUST REPAIR.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. FLOOR OF EMPLOYEE COAT CLOSET, MUST CLEAN.

OBSERVED FOOD NOT PROTECTED FROM CONTAMINATION DURING STORAGE AND DISPLAY. OBSERVED LOAVES OF BREAD, CAKE, CROISSANTS AND CANELES STORED ON TOP OF THE DISPLAY CASES WHERE THEY WERE EXPOSED TO PUBLIC HANDLING AND DROPLET INFECTION. MANAGEMENT INSTRUCTED TO MOVE THE PRODUCT TO COVERED DISPLAY CASES OR TO AN AREA BEHIND THE FRONT COUNTER. SERIOUS VIOLATION 7-38-005A.
