The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. GROUT LOOSE FLOOR TILES IN BASEMENT PREP AREA.
The walls and ceilings shall be in good repair and easily cleaned. SEAL OPENINGS ALONG WALL TILES AND BASEBOARDS, AROUND PIPE FITTING ALONG WALL UNDER 3-COMP SINK IN BASEMENT PREP AREA. WALLS NEED REPAIR UNDER LIQUOR STORAGE IN BAR AREA (WATER DAMAGE, RUST, ETC) REPLACE MISSING FAN COVER IN BASEMENT WASHROOM.
Shielding to protect against broken glass falling into food shall be provided for all artificial lighting sources in preparation, service, and display facilities. OBSERVED 30-40 MICE DROPPINGS ON FLOOR ALONG WALLS LEDGES AND CORNERS IN BASEMENT ELECTRICAL ROOM. 15-20 MICE DROPPINGS ON SHELVES INSIDE LIQUOR STORAGE CABINET IN BAR AREA. INSTRUCTED MANAGER TO CALL AN EXTERMINATOR FOR SERVICE, CLEAN AND SANITIZE ALL AREAS. NO PEST CONTROL LOG BOOK AVAILABLE DURING INSPECTION. LOG BOOK MUST INCLUDE COPY OF PEST CONTROL LICENSE, MSDS SHEETS OF CHEMICALS USED AND MAP OF TREATED AREAS. SERIOUS VIOLATION 7-38-020 CITATION ISSUED.
