OBSERVED A NEW 2 DOOR COOLER AT 68*F ON THERMOMETER COOLER NOT IN USE ALL RACKS STILL TAPED TOGETHER COOLER TAGGED INSTRUCTED MUST BE ABLE TO MAINTAIN POTENTIALLY HAZADROUS FOODS 40*F OR BELOW MUST REPAIR OR REPLACE. TO REMOVE TAG FAX LETTER TO CDPH 1-312-746-4240. NO CITATION ISSUED.
All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. NEED TO CLEAN HOT HOLDING UNIT.

OBSERVED CEILING VENTS OVER MILK COOLER, AND FRONT SERVING LINE DUSTY INSTRUCTED TO CLEAN REMOVE DUST. NEED TO REPLACE ALL WATER DAMAGED CEILING TILES ABOVE WALK-IN FREEZERS. REPLACE MISSING TILE IN REAR STORAGE AREA.

REPLACE STAINED CEILING TILES IN KITCHEN. SEAL OPENINGS AROUND PIPE FITTINGS IN LUNCHROOM STAFF WASHROOM. CLEAN CEILING VENTS IN LUNCHROOM WASHROOM.
REPAIR LEAKING FAUCET ON SINK IN LUNCHROOM STAFF WASHROOM. PROVIDE HOT WATER AT RIGHT SINK IN GIRLS FIRST FLOOR WASHROOM. INCREASE PRESSURE ON SECOND FLOOR SOUTH LEFT FOUNTAIN.
