The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. OBSERVED FOOD/GREASE BUILD UP ON FLOOR UNDER COOKING EQUIPMENT ALSO OBSERVED DAMAGED FLOOR TILES AROUND DISHMACHINE (LOW FLOOR GROUT). INSTRUCTED MANAGER TO CLEAN FLOOR UNDER AND AROUND ALL EQUIPMENT AND REPAIR/REGROUT DAMAGED FLOOR TILES.
The walls and ceilings shall be in good repair and easily cleaned. MUST CLEAN ALL UNCLEAN CEILING LIGHT COVERS ABOVE KITCHEN/DISHMACHINE.
Shielding to protect against broken glass falling into food shall be provided for all artificial lighting sources in preparation, service, and display facilities. OBSERVED ONE CRACKED/SMALL HOLE IN CEILING LIGHT COVER ABOVE DISHMACHINE. MUST REPLACE DAMAGED COVER.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. DISHMACHINE BOOSTER PIPE LEAKING. MUST REPAIR LEAK.
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. PREP AREA COOLERS DOOR RUBBER GASKETS RIPPED AND PREP AREA CUTTING BOARDS SURFACE WITH DARK PITS. MUST REPAIR AND MAINTAIN ALL DAMAGED COOLER DOOR GASKETS AND REPLACE DAMAGED CUTTING BOARDS.
All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. THE FOLLOWING NOT CLEAN- OBSERVED ACCUMULATED FOOD DEBRIS INSIDE PREP COOLERS DOOR HINDGES AND CORNERS, EXTERIOR OF DISHMACHINE WITH FOOD BUILD UP, VENTILATION HOOD SPRINKLERS GREASY AND INTERIOR OF DUMB WAITER WITH ACCUMULATED FOOD BUILD UP. MUST CLEAN AND MAINTIAN ALL UNCLEAN EQUIPMENT.
