All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. CUTTING BOARDS IN NEED OF REPLACING. WORN, DISCOLORED, WITH DEEP CUTS. MUST REPLACE AND MAINTAIN.
The walls and ceilings shall be in good repair and easily cleaned. Use dustless cleaning methods. CEILING AREA IN SERVICE BAR IN NEED OF CLEANING TO REMOVE DUST BUILD-UP. MUST CLEAN AND MAINTAIN.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. LEAK AT HANDSINK IN SERVICE BAR AREA. MUST CORRECT AND MAINTAIN.
Between uses and during storage ice dispensing utensils and ice receptacles shall be stored in a way that protects them from contamination. ICE UTENSILS MUST BE STORED IN A CLEAN SANITIZED CONTAINER.
All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. ICE MACHINE(S) AT 2ND FLOOR IN NEED OF DETAIL CLEANING TO REMOVE BUILD-UP. MUST CLEAN AND MAINTAIN.

The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. FLOORS THROUGHOUT IN NEED OF CLEANING TO REMOVE FOOD SPILLS.
All rooms in which food or drink is prepared, or in which utensils are washed, shall be lighted so that a minimum of 50-foot candles of light is available on all work surfaces. Shielding to protect against broken glass falling into food shall be provided for all artificial lighting sources in preparation, service, and display facilities. ALL LIGHTS IN NEED OF SHEILDING TO ELIMINATE POSSIBILITY OF BROKEN GLASS.
All food establishments that display, prepare, or store potentially hazardous foods shall have calibrated metal stem thermometers, provided and conspicuous, for refrigerated and hot food units. ALL COOLERS IN NEED OF THERMOMETERS. MUST PROVIDE.

RINSE GAUGE OF DISHMACHINE IS NOT WORKING-MUST REPLACE. ALL ICE BINS AT THE BARS MUST HAVE LIDS. RUSTY SHELVES AT WALK-IN COOLER MUST BE RE-SEALED. All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair.
POOR FLOOR GROUTING AT COOK'S LINE & DISHROOM MUST BE REPLACED. FLOOR DRAIN AT BAR (2nd FLOOR) NEED CLEANING. ALL FLOORS MUST BE KEPT DRY AT ALL TIMES. The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair.
WALL COVINGS MISSING BY COFFEE STATION, AT THE GARBAGE ROOM (RIGHT WALL)-MUST BE REPLACED. SOME CEILING TILES IN THE DISHROOM & KITCHEN AREA MUST BE CLEANED/REPLACED-DUE TO DUST OR FOOD STAINS. The walls and ceilings shall be in good repair and easily cleaned.
FOLLOWING PLUMBING ISSUES MUST BE ADDRESSED: FAUCET LEAK AT WASHERS OF HANDSINKS IN KITCHEN & BAR SINKS. GREASE BUILD-UP ON PIPE AT VENTILATION HOOD, INSTRUCTED TO REMOVE AND MAINTAIN.
STANDING WATER ALONG WALL BASE, MISSING GROUT BETWEEN KITCHEN FLOOR TILES AND GREASY SURFACES BEHIND COOKING EQUIPMENT, INSTRUCTED TO GROUT AND MINIMIZE STANDING WATER.
CONDENSATION ON WALK-IN COOLER PIPES WHERE INSULATION IS MISSING, INSTRUCTED TO INSULATE PIPES TO PREVENT CONDENSATION.
