NO PEST CONTROL LOG ON SITE. MUST PROVIDE SAME, LOG BOOK MUST INCLUDE A MAP OF PREMISES, CURRENT STATE LICENSE FOR BOTH THE COMPANY AND THE TECH. COPY OF PESTICIDES/MSDS.
The area outside of the establishment used for the storage of garbage shall be clean at all times and shall not constitute a nuisance. OUTSIDE GREASE DUMPSTER LID NOT CLEAN, CLEAN SAME.
Containers and utensils shall be inverted, covered, or otherwise protected from contamination until used. All single-service drinking straws and containers shall be discarded immediately after use. DETAIL CLEAN INSIDE/OUTSIDE OF ALL EQUIPTMENT.

CLEAN SURFACE UNDER STOVE AND GRILL AND EXTERIOR OF FRYERS WHERE NEEDED. REPAIR OR REPLACE LOOSE RUBBER GASKETS AT DOORS OF COOLER ACROSS FROM STOVE.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. CLEAN FLOOR DRAINS UNDER SINKS WHERE NEEDED. CLEAN FLOORS UNDER COOKING EQUIPMENT TO REMOVE GREASE AND FOOD PARTICLES.
The walls and ceilings shall be in good repair and easily cleaned. CLEAN WALLS AROUND AND BEHIND EQUIPMENT WHERE NEEDED AND REMOVE DUST FROM CEILING VENTS IN KITCHEN WHERE NEEDED.
