All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines. OBSERVED EVIDENCE OF MICE (20 DROPPINGS, LIVE FLIES AND FRUIT FLIES ON PREMISES. HANSEN SERVICED ON AUG. 27, 2010 AND STATED NO ACTIVITY. ADVISED TO HAVE HANSEN SERVICE FACILITY PRIOR TO REINSPECTION NEXT WEEK. MUST ELIMINATE ACTIVITY NOTED. All garbage and rubbish containing food wastes shall, prior to disposal, be stored in metal containers with tight fitting lids and shall be kept covered except when opened for the disposal or removal of garbage. OBSERVED BARRELS OF GREASE WITHOUT COVERS IN REAR STORAGE ROOM ON CARDBOARD BOXES WITH FOOD CRUMBS FROM FISH. INSTRUCTED ON PROPER STORAGE OF USED GREASE. MANAGER STATES THAT AN OUTDOOR GREASE RECEPTOR WILL BE INSTALLED IN A FEW DAYS. INSTRUCTED THAT CURRENT STORAGE ATTRACTS FLIES AND RODENTS. MUST PROVIDE A PROPER GREASE DISPOSAL UNIT. All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. MUST CLEAN THE FOLLOWING AND MAINTAIN: BOTTOMS OF FRYERS TO REMOVE GREASE, EXTERIOR OF USED AND UNUSED EQUIPMENT IN REAR STORAGE ROOM, INTERIORS OF REFRIGERATION UNITS AND FREEZERS, REPLACE LOOSE RUBBER GASKETS AT COOLER AND FREEZER DOORS WHERE NEEDED, AND REPLACE CUTTING BOARDS IN REAR PREP AREA THAT ARE HEAVILY STAINED AND WORN. The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. MUST CLEAN FLOORS AROUND, UNDER AND BEHIND ALL EQUIPMENT AND MAINTAIN. The walls and ceilings shall be in good repair and easily cleaned. REPLACE ALL WATER STAINED CEILING TILES WHERE NEEDED. CLEAN WALLS BEHIND COOKING EQUIPMENT. REPLACE BLOWN BULBS IN WASHROOMS TO PROVIDE LIGHT. Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. REPAIR LEAKING FAUCET NECK AT THREE COMP SINK. All REFRIGERATION UNITS AND FREEZERS NEED INTERNAL THERMOMETERS; PROVIDE. All parts of the food establishment and all parts of the property used in connection with the operation of the establishment shall be kept neat and clean and should not produce any offensive odors. STANDING USED MOP WATER IN REAR PREP AREA NEXT TO THREE COMP SINK NEEDS REMOVING; INSTRUCTED TO REMOVE AFTER USING WATER AND TO HANG MOP PROPERLY TO DRY. OBSERVED CLUTTER OF OLD EQUIPMENT AND OTHER ITEMS IN STORAGE AREAS; ADVISED TO REMOVE UNNECESSARY ITEMS.
