INSTRUCTED TO REMOVE RUST PAINT RACKS WITH NON TOXIC PAINT. RUSTY RACKS IN THE DRY GOODS STORE ROOM AND GREASE TRAP UNDER THE 3 COMPARTMENT IS RUSTY. INSTRUCTED REMOVE RUST AND PAINT SAME.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. THE FLOOR HAS PEELING PAINT IN THE PREP AREA AND STORE ROOMS. INSTRUSTED TO PAINT SAME.
The walls and ceilings shall be in good repair and easily cleaned. THE WEST WALL BEHIND THE STOVE AND PREP TABLE NOT CLEAN HAS FOOD DEBRIS ON IT. INSTRUCTED TO CLEAN SAME.

OVER 10 LIVE COCKROACH'S WERE FOUND IN THE CABINETS BELOW THE SERVING LINE IN THE KITCHEN, AREAS OF INSECT FECAL BUILDUP WERE FOUND IN THE UPPER REACHES OF THE CABINETS AND BEHIND PAPER SIGNAGE TAPED TO THE WALLS. QUALITY & EXCELLENCE PEST CONTROL TREATED THE CAFETERIA ON 06/15/2012. REMOVE ALL EVIDENCE OF INFESTATION AND FILL IN ALL GAPS IN THE UTILITY OPENINGS IN THE KITCHEN. CLEAN UP ALL OF THE DEAD ROACH BODIES UNDER THE THREE COMPARTMENT SINK AND CHECK THE CEILINGS OF THE SERVING AREA ON A CONTINUAL BASIS. OVER 20 MOUSE DROPPINGS WERE FOUND INSIDE OF THE REGISTER AREA TABLES, REMOVE ALL MOUSE DROPPINGS AND SANITIZE THE AREA.
THE KITCHEN FLOORS UNDER THE THREE COMPARTMENT SINK AND STOVE ARE DIRTY, CLEAN THE FLOORS IN DETAIL.
THE WALLS AND BASEBOARDS HAVE GAPS IN THE KITCHEN AND CULINARY ROOM KITCHEN, SEAL UP ALL POSSIBLE PEST HARBORAGE AREAS.
