The walls and ceilings shall be in good repair and easily cleaned. WALLS AND BASEBOARD WALL TILES THROUGHOUT REAR COOKS LINE AND THREE COMPARTMENT SINK CRACKED, MISSING AND IN POOR REPAIR. MUST REPLACE/REPAIR.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. MISSING FLOOR TILES UNDER BATTER STATION. LOOSE FITTING FLOOR DRAIN COVERS THROUGHOUT REAR PREP.
The flow of air discharged from kitchen fans shall always be through a duct to a point above the roofline. Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. THREE COMPARTMENT SINK IN POOR REPAIR. RUSTED NOZZLES AND ATTACHED PIPING. GREASE TRAP ON SINK RUSTED. VENTILATION IN EMPLOYEE WASHROOM NOT OPERATIONAL. LEAK UNDER WASHROOM WASHBOWL SINK. MUST REPAIR/REPLACE ALL.
Toilet rooms shall be completely enclosed and shall be vented to the outside air or mechanically ventilated. WASHROOM DOOR UNABLE TO CLOSE AUTOMATICALLY DUE TO RAISED FLOOR DRAIN COVER. DOOR MUST BE SELF-CLOSING.
All food shall be protected from contamination and the elements, and so shall all food equipment, containers, utensils, food contact surfaces and devices, and vehicles. INTERIOR UPPER PANEL WATER RESERVOIR DIRTY WITH BLACK/BROWN ACCUMULATION DRIPPING INTO WATER WHERE ICE IS MADE. MACHINE TAGGED HELD FOR INSPECTION AND MUST NOT USE UNTIL CLEANED, SANITIZED AND C.D.P.H. CONTACTED AT 312-746-5426 FOR INSPECTION AND TAG REMOVAL.
All food establishments that display, prepare, or store potentially hazardous foods shall have calibrated metal stem thermometers, provided and conspicuous, for refrigerated and hot food units. INACCURATE INTERNAL THERMOMETER IN LINE TALL REACH-IN COOLER. MUST REPLACE.

The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. DETAIL CLEAN THE FLOOR OF THE PREP AREA AROUND WALLS AND UNDER EQUIPMENT.

DETAIL CLEAN THE LEGS OF THE COOKING EQUIPMENT, FAN COVER IN THE POULTRY WALK-IN COOLER, WIRE STORAGE RACK NEAR THE 3 COMPARTMENT SINK.
REPLACE ANY BROKEN FLOOR TILES UNDER THE FRYING EQUIPMENT AND REGROUT THE FLOOR TILES WHERE IT IS MISSING AROUND THE FRYING EQUIPMENT. DETAIL CLEAN THE GREASE FROM THE FLOOR UNDER THE FRYING EQUIPMENT.
