OBSERVED ICE CREAM PREP COOLER AT AMBIENT TEMPERATURE OF 53F, WITH FOODS SUCH AS ASSORTED FRUITS FOR SMOOTHIES AT IMPROPER TEMPERATURE OF 46.9F. COOLER TAGGED HELD FOR INSPECTION AND MUST NOT USE UNTIL REPAIRED, MAINTAINS TEMPERATURE OF 40F OR BELOW AND FAX REQUEST TO C.D.P.H. @ 312-746-4240 FOR RE-INSPECTION AND TAG REMOVAL.
All cold food shall be stored at a temperature of 40F or less. OBSERVED IN THE ABOVE MENTIONED COOLER APPROXIMATELY 3 1/2 GALLONS ASSORTED FRUITS FOR SMOOTHIES AT IMPROPER TEMPERATURE OF 46.9F. ALL FRUIT DISCARDED.
One copy of the Food Inspection Report Summary must be displayed and visible to all customers. SUMMARY REPORT GIVEN BY C.D.P.H. ON 11-23-09 NOT POSTED VISABLE TO CUSTOMERS. FOUND REPORT COVERED.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. ICE CREAM PREP EXPOSED HAND SINK WITH NO HOT OR COLD RUNNING WATER AND SEVERE LEAK UNDER SINK. MUST REPAIR AND PROVIDE HOT AND COLD RUNNING WATER.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. FLOOR OF BOTH WALK IN COOLER AND FREEZER WITH FOOD SPILLAGE. MUST CLEAN AND MAINTAIN. FLOOR OF INDOOR GARBAGE CORRAL WITH DIRT, LIQUID SPILLAGE. MUST CLEAN AND MAINTAIN.
Shielding to protect against broken glass falling into food shall be provided for all artificial lighting sources in preparation, service, and display facilities. MUST PROVIDE LIGHT SHIELDS OR SHATTER PROOF BULBS ABOVE THE SANDWICH PREP STATION.
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. PLASTIC DOOR CURTAINS OF WALK IN FREEZER DIRTY WITH FOOD DEBRIS. MUST CLEAN AND MAINTAIN.

OBSERVED SEWAGE ODOR AT ICE CREAM PREP AREA WHERE GREASE INTERCEPTOR IS LOCATED IN THE FLOOR. 2 SCREWS FOR THE TRAP ARE MISSING FROM THE STEEL LID. MUST INSTALL.
DETAIL CLEAN THE INTERIOR OF THE PEPSI 2 DOOR COOLER IN THE STORAGE ROOM, WIRE STORAGE RACKS IN THE WALK-IN COOLER.
DETAIL CLEAN THE FLOOR OF THE WALK-IN FREEZER AND DRY STORAGE AREA. REPLACE BROKEN FLOOR TILES UNDER THE SANDWICH PREP COOLER.
MUST PROVIDE LIGHT SHIELDS OR SHATTER PROOF BULBS ABOVE THE SANDWICH PREP STATION.
