Sanitizing rinse at three compartment sink not proper cconcentration. Three compartment sink in use and set-up, sanitize rinse concentration at 0ppm. Instructed that sanitzing rinse must be 100ppm chlorine to proper sanitize equiment and utensils. Critical citation issued 7-38-030.

Found interior of ice machine in rear locked area with fresh ice being made, dispensed and stored with mold on interior panel on ledge in contact with fresh ice. Toilet room not maintained or convenient. Toilet rooms both men's and women's not provided with paper products-tissue or disposal hand towels. Instructed that toilet facilities must have paper product at all times. Critical citation issued 7-38-030.
