The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. INSTRUCTED TO REPAIR DAMAGED AND MISSING FLOOR TILES AT FRONT OF 2COMP.SINK,SODA SYRUP STORAGE ROOM AND IN DRY STORAGE ROOM BY DISH MACHINE.FLOORS IN DRY STORAGE ROOM BY ICE MACHINE WITH DEBRIS NEEDS CLEANING.
The walls and ceilings shall be in good repair and easily cleaned. WALLS AT 2COMP.SINK BY DRAIN BOARD BADLY STAINED,INSTRUCTED TO CLEAN.

ALSO OBSERVED LIVE COCKROACH ACTIVITY AT SAME AREA, ABOUT 8 ROACHES NOTED UNDER SHEVES, AND REAR EXIT DOOR NOT RODENT PROOFED, DOOR HAS GAP ON BOTTOM AREA.
