The area outside of the establishment used for the storage of garbage shall be clean at all times and shall not constitute a nuisance. NO OUTSIDE GARBAGE DUMPSTER. INSTRUCTED TO PROVIDE SAME. ALSO PROVIDE THIS ESTABLISHMENTS NAME AND ADDRESS ON THE SIDES OF THE DUMPSTER.
All utensils shall be thoroughly cleaned and sanitized after each usage. CLEAN AND SANITIZE THE THREE COMPARTMENT SINK IT WAS USED FOR A MOP SINK.INSTRUCTED TO ONLY USE THREE COMPARTMENT SINK TO WASH RINSE AND SANITIZE UTENSILS AND POT AND PANS.
ALL FLOORS THROUGHOUT NOT CLEAN.PROVIDE COVING/BASEBOARDS IN TOILET ROOMS AND WHERE NEEDED.INSTD. TO CLEAN ALL FLOORS.
Use dustless cleaning methods. The walls and ceilings shall be in good repair and easily cleaned.SEAL ALL HOLES IN THE WALLS THROUGHOUT PREMISES SO NO INSECT OR RODENT HARBORAGE.
All rooms in which food or drink is prepared, or in which utensils are washed, shall be lighted so that a minimum of 50-foot candles of light is available on all work surfaces.PROVIDE LIGHT BULBS AND LIGHT SHIELDS TO LIGHTS LOCATED UNDER HOOD.REPAIR LIGHTS THAT ARE NOT WORKING IN THE CLASSROOMS.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. THE VENTED HOOD ABOVE THE COOKING EQUIPMENT NOT WORKING. INSTD. TO REPAIR IT. THE WEST CHILDRENS TOILET ROOM HAND SINK NOT WORKING PROPERLY.WHEN WATER IS ON DRAINS ON THE FLOOR. INSTD.TO REPAIR SAME. NO UTILITY/MOP SINK ON SITE.INSTRUCTED TO PROVIDE WITH A AIR GAP AND HOT AND COLD RUNNING WATER UNDER CITY PRESSURE.FAUCET AT THE THREE COMPARTMENT SPRAYS WATER ALL OVER WHEN WATER IS ON.ALSO ONE OF THE COMPARTMENTS IS STOPPED UP.ROD IT OUT AND ROD OUT THE GREASE TRAP UNDERNEATH THE THREE COMPARTMENT ALSO.
