Toilet rooms shall be completely enclosed and shall be vented to the outside air or mechanically ventilated. MUST PROVIDE SELF-CLOSING DEVICE ON EAST SIDE LADIES WASHROOM AND EMPLOYEE WASHROOM IN CHAIR STORAGE ROOM.
The walls and ceilings shall be in good repair and easily cleaned. MISSING TILE AT WEST SIDE LADIES WASHROOM WASH BASIN. WALL BEHIND WIRE RACK SHELVING IN KITCHEN AND CEILING WITH FOOD SPLATTER, MUST CLEAN. LEAKING CEILING ABOVE 1ST FLR. ENTRANCE TO WALK IN COOLER, MUST REPAIR.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. MUST ELEVATE ALL STORED ITEMS IN PATIO BAR FROM FLOOR 6' OR ABOVE. REMOVE POOLING WATER FROM ENTRANCE TO 1ST FLR. WALK IN COOLER. MUST REPLACE MISSING FLOOR TILES IN FRONT DINING ROOM. DEBRIS BEHIND TALL REACH IN FREEZER MUST CLEAN.
Shielding to protect against broken glass falling into food shall be provided for all artificial lighting sources in preparation, service, and display facilities. PROVIDE ABOVE BASEMENT BOX AND WALK IN FREEZER.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. MUST INSTALL DRAIN LINES FROM ALL BAR GUN HOLDERS TO FLOOR DRAINS.
All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. INTERIOR COOKS LINE COOLER MUST CLEAN. COOKS LINE EQUIPMENT WITH EXCESSIVE GREASE, MUST REMOVE, CLEAN.
All parts of the food establishment and all parts of the property used in connection with the operation of the establishment shall be kept neat and clean and should not produce any offensive odors. EMPLOYEE PERSONAL CLOTHING MUST NOT BE STORED IN FOOD AREAS.

OBSERVED LIVE FRUIT FLIES ON THE WALLS AND CEILING IN THE PREP AREA, BAR AND WASHROOMS. 15 LIVE FRUIT FLIES WERE OBSERVED THROUGHOUT THESE AREAS. ADDITIONAL PEST CONTROL SERVICE IS NEEDED TO MINIMIZE OR ELIMINATE THE PEST ACTIVITY. SERIOUS VIOLATION 7-38-020.
