FOUND SERVING ICE IN CONTAINER IN THE BAR NOT PROTECTED DURING SERVICE AND DISPLAY.THE ICE USE FOR COOLING SODA PIPES,SAME ICE WAS LEFT OPEN UNDER DECADE ROTTEN MOLDY WOOD COUNTER, AT THE SAME TIME FOODSERVICE EMPLOYEES ARE SCOOPING THE SERVING ICE WITH THE SERVING CUP INSTEAD OF ICE SCOOP.CITATION ISSUED 7-38-005(a) SERIOUS. INSTRUCTED OWNER TO PROVIDE A SEPARATE CONTAINER WITH LID (COVER) FOR SERVING ICE,AND A SEPARATE ONE FOR SODA PIPES,AND USE ICE SCOOP FOR SERVING THE ICE AND NOT SAME CUP USED FOR SERVING DRINKS TO CUSTOMERS. OBSERVED OVER 20 LIVE SMALL WHITE INSECTS CRAWLING ON FLOOR AND ON WOOD PANEL ADJACENT TO BASEMENT 3-COMPARTMENT SINK GREASE TRAP. INSTRUCTED MANAGER TO REMOVE ALL INSECT ACTIVITY BY DATE OF REINSPECTION (10/03/12).
THE FOLLOWING NOT CLEAN- OBSERVED FOOD DEBRIS ON SHELVES AT BOTH PREP AREAS AND BAR AREA, SLIGHT CALCIUM BUILD UP INSIDE ICE MACHINE. MUST CLEAN AND MAINTAIN EQUIPMENT.
