The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. FLOORS IN NEED OF CLEANING OR REPAINTING WHERE SCRATCHED. MUST CLEAN AND MAINTAIN AT ALL TIMES.

FLOORS ALONG WALLBASES AT PREP AREA/DISHROOM MUST BE FREE FORM FOOD DEBRIS. The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair.
FOLLOWING PLUMBING ISSUES MUST BE ADDRESSED: LEAK AT BOTTOM OF FAUCET HEAD IN PREP AREA & LEAKS AT PIPES UNDER 3-COMP SINK. Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health.
INTERIOR OF 1-COMP. SINK MUST BE KEPT CLEAN & FREE OF DEBRIS/DIRT. DUSTY BATHROOM VENT MUST BE KEPT CLEAN ALSO. All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily.
